| Please provide our office with your most current insurance information. All charges incurred will be filed to your insurance carried. While we participate with most major health insurance companies, check with us when making an appointment regarding our participation status with your insurer. Some insurance companies require that you have a referral for your appointment before you visit with us. Please determine in advance if such referrals are required by your insurance company. While we file all of your charges to your insurance carrier, please keep in mind that our agreement is with you, and you are responsible to pay all co-payments and/or deductibles as required for those services. |
